Infirmary Hospice Care offers support and medical attention to patients who are terminally ill, while allowing them to live in the familiar, comfortable environment of their own homes.
The Hospice team of caregivers includes the patient's personal physician, registered nurses, home health aides, social workers, a chaplain, and volunteers. Team members are prepared to deal with the patient's and the family's needs. Together, they establish a plan of care to provide support for everyone affected by the terminal illness. Hospice care is reimbursed under Medicare and Medicaid plans and most private insurance plans.
Patients can be referred to hospice care by a physician, social worker, or family member.
